General Release of Liability:
Being aware that there is a risk of injury involved whenever children are involved in physical activity, I agree to release and hold Miss Pam Productions, Inc., d/b/a Showtime U.S.A., Ka’Dee S. Carter it’s officers, agents and employees, harmless from any damages or physical injury that is incurred while my child is involved with Showtime U.S.A. attending classes and performances. I accept the risk of and agree to hold Miss Pam Productions, Inc. Showtime U.S.A., USA Camps and Tours, and Pamela G. Masters, its officers, agents and employees, The Carter’s LLC, d/b/a Showtime U.S.A., Ka’Dee S. Carter, its officers, agents, and employees, harmless from all responsibility for damages or physical injury incurred in transporting my child to and from class and performances.
Permission is hereby given for any pictures and/ or videos taken of my child to be used for publicity- including but not limited to: newspaper, websites, YouTube, Facebook, Instagram, Twitter, all social media platforms, etc.
Summer Tour Terms
Please Note: Summer Tour is MANDATORY for all Showtime Cast Members. All Main Street, Kajun Kids, and Singing Star Cast Members are required to have a Parent/ Guardian on tour with them.
Payment of Summer Tour Fees:
Payment of Summer Tour fees are due at the time of billing and shall be paid by check, cash, or debit/ credit card by the 5th day after the billing date.
If unpaid by the 5th day after the billing date, I hereby authorize Showtime USA to charge the credit/ debit card that has been provided. Credit/ Debit card payments will be assessed an additional 3.75% Convenience Fee (minimum charge of $3.75).
A late fee of $15.00 will be assessed on the 11th day after the billing date. A charge of $35.00 will be charged for all returned checks.
Students will not be allowed to attend any class(es) if tour remains unpaid after the 5th day after the billing date.
Tour Payments:
The initial deposit of $200 per seat was included within the five installments paid.
**Late tour payments will result in cast member(s) not being able to attend classes and will be assessed a late fee of $15.00 per installment payment until tour payments are made current.
Tour payments will be billed and due as follows:
February 1 : $125.00 per seat reserved
March 1 : $125.00 per seat reserved
April 1 : $100.00 per seat reserved (along with any balance for other seats are due at this time)

Tour Opt-Out Policy:
As stated, Summer Tour is mandatory for ALL cast members; however, if any cast member wishes to Opt-Out of tour, they may do so by paying an Opt-Out fee of $250.00. The Opt-Out fee is to be paid as follows: $250.00 due at the time of the January Payment.
Cancellation/ Refund Policy:
Showtime USA and USA Camps & Tours reserves the right to cancel its Summer Tour at any time. If Showtime USA/ USA Camps & Tours cancels a tour, all payments made to date of the cancellation will be refunded.
A Cancellation Fee of $250.00 per seat reserved shall be assessed if cancellation is made by the Parent/ Guardian of any cast member after the January payment due date and before February 10th. The said cancellation fee is equal to the tour OptOut fee, which is a non-refundable deposit should the Parent/ Guardian of any cast member cancel at any time before the February 1st payment date. No Credit or Refund is available if the Parent/ Guardian of any cast member cancels after February 1st; if you do not show up; or if you leave tour early for any reason.
No refund will be available if you attend a tour and are dissatisfied with its content.
Please Note: All cancellations must be made in writing by the Parent/ Guardian of the Cast Member. Verbal cancellations or cancellations made by Cast Members are not allowed.

I hereby authorize Miss Pam Productions, Inc. d/b/a Showtime U.S.A., USA Camps & Tours and The Carter’s LLC, d/b/a Showtime U.S.A., Ka’Dee S. Carter, its officers, agents, and employees to charge the above credit/ debit card in compliance with the following:
Payment of Annual & Monthly Membership Membership/ Registration Fees:
Payment of the annual and monthly membership/ registration fees are due at the time of billing and shall be paid by check, cash, or debit/ credit card by the 5th day after the billing date. If unpaid by the 10th day after the billing date, I hereby authorize Showtime USA to charge the credit/ debit card that has been provided above, which shall remain private and confidential.
Credit/ Debit card payments will be assessed an additional 3.75% Convenience Fee (Minimum Fee of $3.75). A late fee of $15.00 will be assessed after the 10th of each month. A charge of $35.00 will be charged for all returned checks. Students will not be allowed to attend any class(es) if tuition remains unpaid after the 10th of each installment due date as shown below. Students must be in the class prior to a show to be staged for that show.
All membership fees will continue to be assessed and billed as set forth above until such time that I give written notice of withdrawal from membership in Showtime USA. It is understood that an annual membership fee, to help offset the cost for production expenses, recording studio fees, and choreography fees will be billed to my account at the time of registration and Five (5) installments will be billed to my account as stated below. These membership fees do not include nor provide tickets or entrance fees to any performances/ productions that require a paid entrance fee/ ticket.
Note: Membership fees are not pro-rated. Showtime USA year runs from August through June of each year; full tuition is due for each month including the months of December and June. Annual and Monthly membership and registration fees are billed as follows:
